写：{entries:[{title,page,level(0-7),anchor?}], confirm:true}，页码须递增，越级静默钳（adjustedLevels）。落 AI 目录位，原目录无损。回退：{useOriginal:true,confirm:true} 切回原始；{useAI:true,confirm:true} 切回 AI。快照救不了目录。真 dry-run（不带 confirm 回 422 + details.wouldWrite）。

读卡片二进制项 GET
?maxEdge=64–4096（默认 2048）&layers=composite|base(0.42 起，base 省旧 mask 位图，按 occlusionBaseContentPath 用)。摘录/插图/手写回合成图；音视频回原始二进制。MCP mn_call 中图片以 image 内容返回。
